Home Health Occupational Therapist Full Time

Position Overview

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaboration with interdisciplinary teams is essential to optimize patient outcomes.

Key Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function.
• Administer and interpret tests to set functional goals and outcomes.
• Provide written evaluations and plans of care, documenting progress and modifying goals as needed.
• Deliver occupational therapy services according to physician orders.
• Maintain comprehensive documentation, including discharge planning and patient education for home exercises.
• Coordinate with healthcare team members, physicians, and caregivers.
• Supervise relevant personnel and complete assessments such as OASIS where permitted.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an Occupational Therapist.
• Valid CPR certification.
