Home Health Occupational Therapist Full Time

Position Overview Seeking an Occupational Therapist (OT) to perform assessments, develop treatment plans, and deliver therapy interventions aligned with physician orders within a home health setting. The role involves collaborating with interdisciplinary team members, providing updates to optimize patient outcomes, and educating patients and caregivers on discharge plans and home exercises. Key Responsibilities Conduct initial and ongoing evaluations of mobility, coordination, strength, and functional abilities.Administer, interpret tests, and set functional goals with measurable outcomes.Prepare written evaluations and care plans, adjusting as needed, and document progress thoroughly.Deliver occupational therapy services per physician orders, ensuring compliance.Coordinate with healthcare team members, including physicians and caregivers.Participate in discharge planning, providing education and home exercise guidance.Supervise relevant personnel, such as OTA or HHA, where applicable.Complete assessments according to state regulations and maintain accurate documentation to support billing and compliance. Qualifications & Skills Degree in Occupational Therapy from an accredited program.Current, unrestricted state license as an Occupational Therapist.Proficiency in clinical assessments, documentation, and compliance.Valid CPR certification, driver's license, and automobile insurance.
