Home Health Occupational Therapist OT Full Time

Position Overview

Key Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength, and function.
• Administer and interpret tests to set functional goals and outcomes.
• Develop and document plans of care, including goals, progress, and expected results.
• Deliver occupational therapy services per physician orders.
• Maintain documentation, modify treatment plans, and plan discharges.
• Educate patients and caregivers on home exercise and discharge plans.
• Cook with healthcare team and document effectively with minimal supervision.
• Supervise staff such as OTAs and HHAs as applicable.
• Complete OASIS assessments where permitted.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an OT.
• Valid CPR certification.

Preferred Skills

• Experience in home health therapy and OASIS documentation.
