Position Overview The Licensed Practical Nurse provides skilled nursing care following care plans and physicians’ orders, ensuring confidentiality and professional boundaries. Key Responsibilities Deliver quality patient care under the supervision of a Registered Nurse, including documentation and communication with healthcare providers.Recognize patient needs and responses, implementing interventions to prevent or reduce risks.Educate patients and families based on individual needs.Perform home-based care tasks such as wound care, infusions, catheter care, and dressing changes, utilizing a variety of skills. Qualifications & Skills Active LPN license in the applicable stateValid CPR certificationPreferred: experience in Medicare Skilled Nursing, home health, or related fields; basic Oasis knowledge; Home Care Home Base experience is a plus.
