Home Health LPN PRN Part Time

Position Overview: The Licensed Practical Nurse provides skilled nursing care in patients' homes, following care plans and physicians' orders while maintaining confidentiality and professionalism. Responsibilities Deliver quality patient care under RN supervision, documenting and communicating effectively with healthcare team.Recognize and respond to changes in patient needs, implementing appropriate interventions.Educate patients and families based on individual needs.Perform hands-on care such as wound care, infusions, catheter care, and dressing changes. Qualifications & Skills Active LPN license in the applicable state.Valid CPR and driver’s license.Preferred: Medicare Skilled Nursing experience, home health experience, and familiarity with Oasis or Home Base. Key Duties: Provide compassionate, compliant home healthcare, ensuring patient safety, proper documentation, and effective communication with physicians and caregivers.
