Position Overview
The Licensed Practical Nurse provides and documents skilled nursing care following care plans and physicians’ orders, ensuring confidentiality and professionalism.
Responsibilities
- Deliver quality patient care
- Perform hands-on care
- Recognize and respond to patient needs
- Educate clients and families
- Provide home-based care such as assessments, wound care, infusions, and catheter management
Qualifications & Skills
- Must hold an active LPN license
- CPR certification required
- Valid driver’s license preferred
- PREFERRED: Experience in Medicare Skilled Nursing, home health, and familiarity with Home Care Home Base or Oasis
