Part Time Home Health LPN

Position Overview: The Licensed Practical Nurse provides and documents skilled nursing care following care plans and physicians’ orders, ensuring confidentiality and professionalism. Responsibilities: Deliver quality patient care, perform hands-on care, recognize and respond to patient needs, educate clients and families, and provide home-based care such as assessments, wound care, infusions, and catheter management. Qualifications & Skills: Must hold an active LPN license, CPR certification, and a valid driver’s license. Preferred experience includes Medicare Skilled Nursing, home health, and familiarity with Home Care Home Base or Oasis.
