Home Health LPN - Part Time

About the role

The Licensed Practical Nurse provides and documents skilled nursing care aligned with care plans and physicians' orders, maintaining confidentiality and professional boundaries.

Responsibilities

• Deliver quality patient care under RN supervision, documenting and communicating with physicians and caregivers.
• Recognize patient condition changes, implement interventions, and educate patients and families based on needs.
• Perform home care tasks such as wound care, infusions, catheter care, PICC line dressing changes, and phone triage.

Qualifications & Skills

• Active LPN license in the applicable state
• Valid CPR certification
• Preferred: Medicare Skilled Nursing experience, understanding of Oasis, 1+ year LPN experience in home health or healthcare setting, Home Care Home Base experience
