Home Health LPN PRN Part Time

Position Overview: The Licensed Practical Nurse provides and documents skilled nursing care in patients' homes, following care plans and physicians' orders while maintaining confidentiality and professional boundaries. Responsibilities Deliver quality patient care under the supervision of a Registered Nurse, including documentation and communication with caregivers.Recognize patient needs and responses, implementing interventions to reduce risks.Educate patients and families based on individual needs.Perform home care tasks such as wound care, infusions, catheter care, and assessments. Qualifications & Skills Active LPN license in the applicable stateValid CPR certificationPreferred: Medicare skilled nursing experience, knowledge of Oasis, home health experience, and familiarity with Home Care Home Base Key Attributes: Compassionate, detail-oriented, able to communicate effectively, and skilled in various clinical procedures.
