Home Health LPN, North Philadelphia

About the role

We are seeking talented, passionate individuals to join our team and help our patients live happier and healthier lives in their homes. This position will cover the North Philly service area.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Requirements

• Active LPN state license
• Valid driver’s license
• Two or more years of applicable nursing experience

Bilingual (Spanish) preferred.
