Responsibilities
- Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
- May not alter the plan of care.
- Instructs the clients' family in the care of the client and maintenance of a healthy environment for the client.
- Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the client's condition daily.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Qualifications
Required:
- Current and unrestricted LPN licensure in state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Ability to work flexible hours as required to meet identified patient needs.
Preferred:
- Current CPR certification or ability to complete within 90 days of hire.
- Previous home care experience.
- Ability to work flexible hours as required to meet identified patient needs.
- Able to work independently.
- Good communication, writing, and organizational skills.
Pay
Pay range: $49,151 – $73,726 annual total cash target pay; $28.36 – $42.53 per visit point; $23.63 – $35.45 hourly rate. Annual total cash compensation assumes full‑time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per‑visit point pay and hourly pay and is based on factors such as local labor markets, education, work experience, and may increase over time with productivity and performance. The role receives two types of compensation: per‑visit point rate for visits (calculated by multiplying the per‑visit point rate by productivity points accrued for each visit type) and an hourly rate for non‑visit activities such as orientation. Minimum wage laws are complied with as applicable.

Schedule
Full Time or Part Time schedule. Territory coverage: Luzerne County, PA.
