Home Health LPN - Full Time

Position Overview This Licensed Practical Nurse (LPN) role involves delivering and documenting skilled nursing care in accordance with care plans and physicians’ orders, ensuring confidentiality and professional boundaries. Key Responsibilities Provide quality patient care under RN supervision, following physician directives.Perform hands-on client care, document activities, and communicate with physicians and caregivers.Identify patient needs and responses requiring intervention, implementing measures to reduce risks.Educate clients and families based on individual needs.Administer care in patients' homes, including phone triage, wound care, infusions, catheter care, and dressing changes. Qualifications & Skills Active LPN license in the applicable state.Valid CPR certification, automobile insurance, and driver’s license.Preferred: 1-year home health experience, familiarity with Home Care Home Base, and IV certification.
