Seeking a licensed practical nurse responsible for patient assessment, planning, implementation, and evaluation based on age and diagnosis. The role involves collaborating with a multidisciplinary team to deliver effective patient care, communicating in accordance with agency policies, and contributing to program efficiency. The nurse organizes workload efficiently, maintains schedules, and embodies the agency's values daily.

Qualifications

• Graduate of an accredited practical nursing program with current North Carolina licensure and CPR certification.
• Minimum one year of practical nursing experience, preferably in home health.
• Proficient in hand/eye coordination, equipment manipulation, and effective communication in English.
• Physical ability to transfer objects and patients, perform patient care tasks, and maneuver in various settings.
• Strong assessment skills, critical thinking, and ability to prioritize care.
• Sensory acuity for visual and auditory patient assessments.
• Willingness to work variable hours, including weekends and holidays, and serve diverse populations.
• Valid North Carolina driver’s license and operational vehicle required.
