Home Health Licensed Practical Nurse

You’ll work with a dedicated team of physicians and clinicians to manage care plans that support recovery and improve patients' quality of life.
About the role
As a Home Health LPN, you will deliver skilled nursing care to patients in their homes under the direction of an RN. You’ll implement individualized care plans in collaboration with the patient, their family, and the healthcare team, while educating them on disease management, treatment options, and self-care techniques. Maintaining accurate records of patient care, coordinating with physicians and clinical managers, and participating in care coordination and discharge planning are key aspects of this role.
Responsibilities
- Provide skilled nursing care to patients in their homes under RN supervision.
- Implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including visit documentation, interventions, and outcomes.
- Communicate promptly with patients, physicians, clinical managers, and the healthcare team regarding patient needs and status changes.
- Participate in care coordination activities and discharge planning.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
Preferred Qualifications
- Nursing experience in a Home Health or Hospice setting.
Pay
The pay range for this role is $33.00 - $47.00 per visit/unit, with an annual base pay range of $53,000 - $73,000 for full-time (40 hours per week) employment. Compensation may vary based on geographic location, skills, knowledge, experience, education, and certifications.

Schedule
This is a full-time role with a scheduled weekly commitment of 40 hours.
Additional Information
- This role is patient-facing and requires TB screening as part of Humana’s Tuberculosis (TB) screening program.
- This role is part of Humana’s driver safety program. You must maintain a valid state driver’s license and personal vehicle liability insurance with minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
