Home Health Licensed Practical Nurse
Job Summary
As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you'll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. You'll collaborate with the Registered Nurse and care team to manage medical needs and promote independence.
Qualifications
- Graduate of an accredited LPN/LVN program
- Minimum of one year experience as an LPN/LVN in a clinical care setting
- Home care experience preferred
- Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services
- Mets all additional requirements for hire including state specific requirements
- Current BCLS prior to delivering patient care
- Must have valid driver’s license and reliable transportation
Responsibilities
- Provide nursing care in the home, including wound care, medication administration, and support during procedures
- Document assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
- Monitor patient status and alert the care team of any changes to ensure timely, coordinated responses
- Educate patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
- Collaborate closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting
