Home Health Licensed Practical Nurse

Job Summary

Provides nursing care under the supervision of a registered nurse, utilizing the nursing process (assessment, planning, implementation, evaluation) in a home‑health setting. Assists in developing and implementing individualized care plans for patients across all life stages—from neonates to geriatrics—while adhering to agency policies, procedures, and standards of care. Supports a multidisciplinary team to deliver effective patient management, ensures clear communication, and maintains high customer satisfaction. Demonstrates commitment to agency values, promotes positive interpersonal relations with patients, families, and staff, and maintains flexibility in scheduling while upholding the agency’s mission, vision, and values.

Responsibilities

• Deliver nursing care in patients’ homes following the nursing process.
• Develop and implement individualized care plans for diverse patient populations.
• Collaborate with a multidisciplinary team to coordinate patient management.
• Communicate clearly with patients, families, and staff to ensure continuity of care.
• Maintain high levels of customer satisfaction and professional conduct.
• Adapt scheduling as needed to meet patient and agency needs.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Practical Nursing license in North Carolina.
• CPR certification.
• At least one year of practical nursing experience, preferably in home health.

Technical Skills

• Manual dexterity for patient care tasks.
• Proficiency in handling medical equipment.
• Effective communication skills in English.

Physical Demands

• Lifting, transferring, and repositioning patients safely.
• Assessing patient conditions and formulating appropriate care plans.
• Strong interpersonal skills and ability to prioritize duties.
• Adaptability to various environmental hazards.
• Valid North Carolina driver’s license and access to an operational vehicle.
