About the role
We are seeking a Home Health Licensed Practical Nurse (LPN) to provide personalized, skilled nursing care in patients’ homes. Reporting to the Clinical Manager, you will implement individualized care plans, educate patients and families on disease management and self‑care, and maintain accurate documentation of care and outcomes. Core focus: deliver compassionate home‑based care to support patient recovery and independence.
Responsibilities
- Implement individualized care plans for patients in their homes.

- Collaborate with healthcare teams and participate in care coordination.
- Promptly communicate patient needs and status changes.
- Adhere to TB screening requirements and driver safety policies.
Qualifications
- Valid Licensed Practical Nurse (LPN) license.
- Current CPR certification.
- Minimum two years of clinical experience.
- Reliable transportation and a valid driver’s license.
- Preferred: experience in Home Health or Hospice settings.
Pay
Compensation varies based on skills and location, with a pay range of $53,000 – $73,000 annually, plus a $5,000 sign‑on bonus.
