About the role
A Licensed Practical Nurse (LPN) works under supervision to assist in delivering patient‑centered care in a Home Health setting.
Responsibilities
- Assist with assessment, planning, implementation, and evaluation of patient care across all age groups, including adults, geriatrics, and pediatrics.
- Coordinate with multidisciplinary teams to manage patient care efficiently.
- Communicate effectively following agency policies and discipline‑specific guidelines.
- Maintain organizational efficiency, schedule, and uphold agency values and mission.
- Ensure patient satisfaction and foster positive interpersonal relations with patients, families, and staff.
Qualifications
- Graduate of an accredited practical nursing program with a current North Carolina license.
- CPR certification required.
- Minimum one year of practical nursing experience, preferably in home health.
- Proficiency in patient‑care equipment, strong hand/eye coordination, and effective communication skills.
- Physical ability to perform patient transfers, lifting, and positioning.
- Mental capacity to assess conditions, develop care plans, and educate patients.
- Visual and auditory acuity for patient assessment and communication.
- Valid North Carolina driver’s license and operational vehicle required.
Schedule
Flexibility to work variable hours, including weekends and holidays, serving diverse patient populations.
