Home Health Licensed Practical Nurse
About the Role
As a CenterWell Home Health nurse, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes.
- Maintain prompt contact with patients, physicians, clinical managers, and other members of the healthcare team regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
Preferred Qualifications
- Nursing experience in a Home Health or Hospice setting.
Skills
Patient care, care plan implementation, patient and family education, clinical documentation, care coordination, and communication with healthcare teams.
Pay
- Per Visit/Unit Rate: $33.00 - $47.00
- Annual full-time (40 hours per week) range: $53,000 - $73,000 per year, based on geographic location and individual qualifications.

Additional Information
- This role is patient-facing and requires TB screening as part of Humana’s Tuberculosis screening program.
- This role requires a valid state driver’s license and maintenance of personal vehicle liability insurance. Minimum required limits are $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage or the state’s minimum required limits, whichever is higher.
