Home Health Licensed Practical Nurse

Provide high-quality, coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

Responsibilities

• Administer medications, treatments, and other medical care as prescribed by a physician
• Monitor patient health status and document any changes in condition
• Use clinical judgment to provide effective care to patients
• Collaborate with other healthcare professionals to develop and implement care plans
• Educate patients and their families on proper care techniques and disease management
• Maintain accurate and complete medical records
• Provide emotional support and compassionate care to patients and their families
• Adhere to all applicable laws, regulations, and ethical standards of nursing practice

Requirements

• Current LPN license in the state of Florida
• At least 1 year of experience in home health care or a related field
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong clinical skills and sound judgment
• Familiarity with EMR systems
• Valid FL driver’s license and reliable transportation (auto insurance must also be presented)
• CPR certification (original card required)
• Level 2 Criminal Background Check
