Home Health Licensed Practical Nurse
Join our caring community as a Home Health LPN and provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. You’ll work with a dedicated team of physicians and clinicians to manage care plans that support recovery and improve quality of life.
About the role
As a Home Health LPN, you will deliver skilled nursing care to patients in their homes under the direction of an RN. You’ll implement individualized care plans in collaboration with the patient, family, and healthcare team, and educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including visit documentation, interventions, and outcomes.
- Communicate promptly with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
- Participate in care coordination activities and discharge planning.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
Preferred Qualifications
- Nursing experience in a Home Health or Hospice setting.
Pay
- Per Visit/Unit Rate: $37.00 - $52.00
- Annual full-time (40 hours per week) range: $58,300 - $80,300 (varies by geographic location and individual qualifications).

Additional Information
- This role is patient-facing and requires TB screening as part of Humana’s tuberculosis prevention program.
- This role requires driving; you must maintain personal vehicle liability insurance with minimum limits of $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 for property damage (or higher, as required by your state).
