As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. In Cayuga County, as a Home Health LPN you will provide skilled nursing care to patients in their homes under the direction of an RN.
Responsibilities
- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN
- Implement individualized care plans in collaboration with the patient, family, and healthcare team
- Educate patients and their families on disease management, treatment options, and self‑care techniques
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
Requirements
- Current nursing license in the practicing state
- Valid driver’s license, auto insurance, and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
Preferred Qualifications
- Nursing experience in a Home Health or Hospice setting

Pay
Pay per visit/unit rate: $33.00 – $47.00. Estimated annual base pay range for full‑time (40 hours/week): $53,000 – $73,000 per year.
