Home Health Clinical Support RN - Multiple Locations!!

Key Responsibilities

• Provide all levels of care to the patients
• Support patients in Evanston, IL and surrounding areas
• Meet critical deadlines for charting
• Self-manage time and schedule
• Demonstrate experience with tablets, mobile phones, and EMR software

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of service.
• High School/GED
• Epic Home Health (1+ years)
• 2+ years of experience in ANY of the following: Home Health Care and Home Health
• 1+ years of experience in ANY of the following: Electronic Medical Record (EMR), Other EMR and TruChart EHR
• Valid Registered Nurse (RN) in the state of Illinois (IL)
• Valid Regular Driver License (DL)

Schedule and Work Locations

• Monday through Friday on the day shift; no weekends, on-call or holidays

Hire a Home Health Clinical Support RN to provide clinical oversight, assessment, and intervention for home-based patients with a wide range of care needs. Coordinate services, conduct regular patient evaluations, and ensure adherence to regulatory compliance and company protocols.
