Home Health Clinical Support - RN

About the role

We are seeking a dedicated and compassionate individual with clinical expertise to join our team as a Home Health Clinical Support - RN. This role is pivotal in providing top-notch care to our patients in a home setting, ensuring they receive comprehensive and attentive medical support.

Responsibilities

• Provide clinical support and guidance to patients and their families in a home health setting
• Conduct comprehensive assessments and develop personalized care plans
• Collaborate with a multidisciplinary team to ensure coordinated care
• Monitor and document patient progress and make necessary adjustments to care plans
• Educate patients and caregivers on health management and protocols
• Ensure compliance with all health regulations and company policies

Schedule and Shift Details

Monday through Friday on the day shift; no weekends, on-call or holidays

Qualifications

• Current RN license in the state of practice from IL with DL
• Minimum of 2 years of clinical experience, preferably in home health
• 1+ years of experience in EMR, Other EMR and TruChart EMR
• Excellent communication and interpersonal skills
• Ability to work independently and make informed clinical decisions
• Strong organizational skills and attention to detail
• Proficiency in medical record-keeping and documentation
