Core Responsibilities
- Provide clinical support and guidance to patients and families in a home health setting
- Perform comprehensive patient assessments and create personalized care plans
- Collaborate with a multidisciplinary healthcare team to ensure comprehensive care
- Monitor, evaluate, and document patient progress, adjusting care plans as necessary
- Educate patients and caregivers on health management and medical protocols
- Ensure adherence to health regulations and organizational policies
Required Qualifications
- Current RN license in state of practice
- Minimum of 2 years of clinical experience, preferably in home health
- Experience with Electronic Medical Records (EMR) systems, including TruChart EMR
- Strong communication and interpersonal skills
- Ability to work independently and make informed clinical decisions
- Exceptional organizational skills and attention to detail
- Proficient in medical documentation and record-keeping
Work Schedule
Monday through Friday, day shifts; no required weekends, on-call, or holidays
Coverage area: Glenview, Evanston, Skokie, Niles, Chicago, Park Ridge & surrounding
