What You'll Do
- Get the Best Training: Start with a two-week clinical orientation to learn everything you need to succeed – from key procedures to our EMR system, safety protocols, and compassionate care.
- Be the Expert: Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
- Make a Connection: Educate and guide patients and families through their healthcare journey – helping them understand their conditions and the path to better health.
- Collaborate for Success: Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
- Document with Purpose: Record and track changes in patient conditions, updating care plans to reflect their progress and needs.
Qualifications
- Registered Nurse (RN) with a bachelor's degree in nursing (preferred).
- 1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare!).
- Strong verbal and written communication skills – you'll be guiding and educating patients and families.
- Proficient with technology – navigating EMR systems and using digital tools to enhance patient care.
- A reliable car and valid driver's license – you'll be traveling to patient homes, so your own transportation is key.
Physical Demands/Working Conditions
- Flexibility to work both indoors and outdoors while using clinical equipment.
- Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.
- You'll be making a direct impact through client and customer contact, so your presence is invaluable.
Pay
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $50.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
