About the role
The Care at Home Registered Nurse (RN) supports patients and families across the continuum of the medical home model. As an RN, you will complete in-home assessments, monitoring, care planning, and interventions for acute care patients, using approved technology and clinical protocols in collaboration with the care team. You will deliver exceptional care throughout the patient journey, working directly with patients and families to enhance their experience and provide education as needed.
Responsibilities
- Complete in-home assessments, monitoring, care planning, and interventions for acute care patients.
- Implement, monitor, evaluate, update and revise patient care plans.
- Note and carry out physician and nursing orders.
- Administer prescribed medications and treatments.
- Note and carry out physician and nursing orders.
- Assess and coordinate patients’ discharge needs.
- Deliver exceptional care throughout the patient journey, working directly with patients and families to enhance their experience and provide education as needed.
Requirements
- Graduate from an accredited school of professional nursing (ADN or BSN program).
- Current state licensure as a Registered Nurse (RN).
- Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross.
- Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) or ALS American Red Cross certification must be presented upon hire or within first year of hire based on the department service level RN is hired into.
- One year of current acute care RN experience. Will consider licensed military RN experience.
Qualifications
- Bachelor of Science in Nursing (BSN) preferred.
- Technology experience (EPIC, radiology platforms, teams) preferred.
- Med/Surg or observation experience preferred.
- Three years of experience in a hospital setting preferred.
Skills
- Excellent communication skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Proficiency with electronic health records (EHR) systems.

Schedule
Nights from 6:45 p.m. – 7:15 a.m., with weekend and holiday requirements.
