Home Health Clinical Manager (RN)

About the role

If you’re an experienced Registered Nurse (RN) who’s passionate about leading and making a tangible impact in patient care, we want you on our team. Interim HealthCare is seeking a Clinical Manager to oversee and elevate the patient care services at our agency.

Responsibilities

• Lead and support an interdisciplinary clinical team, ensuring the delivery of high-quality, patient-centered home health care.
• Oversee patient assessments, individualized plans of care, and coordination of physician-ordered services.
• Supervise, mentor, and evaluate clinical staff while promoting regulatory compliance and best nursing practices.
• Monitor clinical documentation, patient outcomes, and quality metrics to ensure accuracy, timeliness, and agency standards are met.
• Collaborate with physicians and the care team to coordinate referrals, transitions of care, and discharge planning.
• Educate and support patients and caregivers to promote understanding of care plans, disease management, and health goals.
• Lead Quality Assessment and Performance Improvement (QAPI) initiatives and conduct supervisory home visits to ensure exceptional care delivery.

Requirements

• Current RN license in South Carolina or compact multi-state license.
• RN license in good standing.
• Graduate of an accredited nursing program.
• PREFERRED: 3+ years of home care experience.
• PREFERRED: 2+ years of management experience.
• Strong background in clinical practice and home care management.

• Comprehensive knowledge of home health care regulations and best practices.
• Excellent communication and interpersonal skills.
• Ability to manage multiple tasks and prioritize effectively.
• Proficiency in Microsoft Office Suite.
