About the role
The Clinical Manager at Sunrise Home Health Services is a leadership position designed for experienced Registered Nurses who want to expand their impact by mentoring clinicians, driving quality outcomes, ensuring regulatory excellence, and shaping the future of the organization.
Responsibilities
- Mentor and support nurses, therapists, and home health aides
- Provide clinical guidance and decision support to field staff
- Aid in onboarding, training, and professional development of clinicians
- Participate in case conferences and interdisciplinary collaboration
- Serve as a resource for complex clinical situations
- Monitor patient outcomes and quality indicators
- Ensure compliance with Medicare Conditions of Participation (CoPs)
- Support PDGM compliance and reimbursement optimization
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
- Conduct audits and identify opportunities for process improvement
- Ensure adherence to state, federal, and accreditation standards
- Assist with clinical scheduling and productivity management
- Support physician communication and care coordination
- Collaborate with referral sources and healthcare partners
- Address patient concerns and service recovery opportunities
- Aid in strategic clinical initiatives and agency growth
Qualifications
- Current Arizona Registered Nurse (RN) license in good standing
- Home Health experience
- OASIS experience
- Strong understanding of Medicare Conditions of Participation (CoPs)
- Knowledge of PDGM regulations and reimbursement models
- Excellent communication, leadership, and organizational skills

$95,000 - $105,000 annually
