Position Summary
To perform intermittent direct or indirect hands-on patient care to meet the patient's needs in accordance with the plan of care and assist in maintaining a safe and clean environment under the direction and supervision of a Registered Professional Nurse.

The position involves providing care in a skilled nursing facility, home health agency, or through demonstrated proficiency. Completion of a State-approved Home Health Aide Training course, competency exam, and certification through the New Jersey State Board of Nursing is required. The role requires working in a changing patient care environment, meeting patient needs, and collaborating with multidisciplinary teams.
Responsibilities
- Perform intermittent direct or indirect hands-on patient care
- Maintain a safe and clean environment
- Assist in implementing the patient's plan of care
- Work cooperatively with patients, families, and multidisciplinary team members
- Utilize effective time management to prioritize tasks and respond to emergencies
- Read, write, and understand English
- Drive a vehicle as needed, adhering to all company policies
Requirements
- High school diploma or equivalent
- One year of current experience in a skilled nursing facility, home health agency, or proven proficiency
- Completion of the State approved Home Health Aide Training course, Home Health Aide competency exam, and certification through the New Jersey State Board of Nursing
- Ability to work in a changing patient care environment and meet patient's needs
- Ability to work cooperatively within the health system and with multidisciplinary team members
- Ability to utilize effective time management to set priorities and respond to emergencies
- Ability to read, write, and understand English
- Driver's License - current and valid driver's license, registration, inspection sticker, and appropriate level of insurance coverage required
