Position Summary
To perform intermittent direct or indirect hands-on patient care to meet the patient's needs in accordance with the plan of care and assist in maintaining a safe and clean environment under the direction and supervision of a Registered Professional Nurse.

The position involves providing care to patients in a skilled nursing facility, home health agency, or similar setting. Responsibilities include assisting with daily living activities, administering medications, monitoring vital signs, and ensuring the cleanliness and safety of the patient's environment.
Responsibilities
- Perform hands-on patient care tasks as directed by a Registered Professional Nurse
- Assist with daily living activities such as bathing, dressing, and eating
- Administer medications and monitor patient responses
- Maintain a clean and safe patient environment
- Communicate effectively with patients, families, and healthcare team members
- Adhere to infection control protocols and safety standards
Requirements
- High school diploma or equivalent
- One year of current experience in a skilled nursing facility, home health agency, or proven proficiency
- Completion of the State approved Home Health Aide Training course, Home Health Aide competency exam, and certification through the New Jersey State Board of Nursing
- Ability to work in a changing patient care environment and meet patient's needs (chronic and/or terminal illness)
- Ability to work cooperatively within the health system; with patient and family members; and with multidisciplinary team members (i.e., nurses, Rehab staff, etc.)
- Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations
- Ability to read, write, and understand English
- Driver's License - current and valid driver's license, registration, inspection sticker, and appropriate level of insurance coverage required
Qualifications
- Completion of the State approved Home Health Aide Training course
- Home Health Aide competency exam and certification through the New Jersey State Board of Nursing
- Proven proficiency in a skilled nursing facility, home health agency, or similar setting
- Ability to work in a changing patient care environment and meet patient's needs (chronic and/or terminal illness)
- Ability to work cooperatively within the health system; with patient and family members; and with multidisciplinary team members (i.e., nurses, Rehab staff, etc.)
