Position Summary

To perform intermittent direct or indirect hands-on patient care to meet the patient's needs in accordance with the plan of care and assist in maintaining a safe and clean environment under the direction and supervision of a Registered Professional Nurse.

Completion of a State-approved Home Health Aide Training course, passing the competency exam, and certification through the New Jersey State Board of Nursing are required. The role requires working in a changing patient care environment, managing patient needs, and collaborating with multidisciplinary teams.

Responsibilities

  • Perform intermittent direct or indirect hands-on patient care
  • Maintain a safe and clean environment
  • Assist in implementing the patient's plan of care
  • Work cooperatively with patients, families, and multidisciplinary team members
  • Utilize effective time management to prioritize tasks and respond to emergencies
  • Read, write, and understand English
  • Drive to work locations as needed

Requirements

  • High school diploma or equivalent
  • One year of current experience in a skilled nursing facility, home health agency, or proven proficiency
  • Completion of the State approved Home Health Aide Training course, Home Health Aide competency exam, and certification through the New Jersey State Board of Nursing
  • Ability to work in a changing patient care environment and meet patient's needs (chronic and/or terminal illness)
  • Ability to work cooperatively within the health system; with patient and family members; and with multidisciplinary team members (i.e., nurses, Rehab staff, etc.)
  • Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations
  • Driver's License - current and valid driver's license, registration, inspection sticker, and appropriate level of insurance coverage required

Qualifications

  • Completion of the State approved Home Health Aide Training course
  • Passing the Home Health Aide competency exam
