About the role

Join Addus HomeCare and help older adults and individuals with disabilities live safely and independently at home. As a Service Coordinator, you’ll lead scheduling, ensure top-quality service, and conduct in-home visits. You’ll be the go-to problem solver—juggling schedules, supporting field staff, handling client updates, and stepping in to keep care plans on track.

Location: Addus HomeCare, 1240 N. Riverside, Medford, OR 97501.

Schedule

  • Full Time, Monday through Friday, 8:00 AM to 5:00 PM (1-hour lunch break)
  • Evenings as needed
  • Weekends as needed

Responsibilities

  • Creates work schedules by entering schedules into the system; manages changes due to client requests, illness, vacation, or leaves of absence.
  • Provides alternate coverage to ensure the client’s care plan is followed and services are not interrupted.
  • Contacts care providers and clients to provide service updates.
  • Conducts monthly client wellness calls and home visits as required.
  • Provides thorough follow-through on escalated client complaints and theft claims.
  • Supervises direct service employees by setting expectations for attendance, performance, and conduct; holds employees accountable to company policies and guidelines.
  • Ensures the branch’s open environment is professional: neat, clean, orderly, and free of clutter.
  • Maintains a high degree of confidentiality due to access to sensitive information.
  • Maintains regular, predictable, consistent attendance and is flexible to meet agency needs.
  • Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abides by all regulations, policies, procedures, and standards.
  • Answers telephones, assists staff, greets visitors, files/scans, prepares reports, conducts file reviews, and performs data entry.
  • Handles special projects as assigned.

Requirements

  • High school diploma or equivalent.
  • 3 to 5 years of industry experience required.
  • Interpersonal, organizational, and communication skills.
  • Computer skills including Microsoft Word, Microsoft Excel, and scheduling programs.
  • Valid driver’s license to complete in-home supervisory visits.
