About the role
Join Addus HomeCare and help older adults and individuals with disabilities live safely and independently at home. As a Service Coordinator, you’ll lead scheduling, ensure top-quality service, and conduct in-home visits. You’ll be the go-to problem solver—juggling schedules, supporting field staff, handling client updates, and stepping in to keep care plans on track.
Location: Addus HomeCare, 1240 N. Riverside, Medford, OR 97501.
Schedule
- Full Time, Monday through Friday, 8:00 AM to 5:00 PM (1-hour lunch break)
- Evenings as needed
- Weekends as needed
Responsibilities
- Creates work schedules by entering schedules into the system; manages changes due to client requests, illness, vacation, or leaves of absence.
- Provides alternate coverage to ensure the client’s care plan is followed and services are not interrupted.
- Contacts care providers and clients to provide service updates.
- Conducts monthly client wellness calls and home visits as required.
- Provides thorough follow-through on escalated client complaints and theft claims.
- Supervises direct service employees by setting expectations for attendance, performance, and conduct; holds employees accountable to company policies and guidelines.
- Ensures the branch’s open environment is professional: neat, clean, orderly, and free of clutter.
- Maintains a high degree of confidentiality due to access to sensitive information.
- Maintains regular, predictable, consistent attendance and is flexible to meet agency needs.
- Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements.
- Abides by all regulations, policies, procedures, and standards.
- Answers telephones, assists staff, greets visitors, files/scans, prepares reports, conducts file reviews, and performs data entry.
- Handles special projects as assigned.
Requirements
- High school diploma or equivalent.
- 3 to 5 years of industry experience required.
- Interpersonal, organizational, and communication skills.
- Computer skills including Microsoft Word, Microsoft Excel, and scheduling programs.
- Valid driver’s license to complete in-home supervisory visits.
