HOME CARE CLINICAL MANAGER - RN

Responsibilities

• Provide coordination and case management of all patient services
• Provide appropriate documentation for both patient care and staff performance
• Supervise assigned care staff
• Assist administrator as necessary
• Provide direct patient care as necessary, including case management, OASIS care planning, assigning care staff, and ensuring efficient, effective utilization of existing staffing resources

Requirements

• Current RN licensure within state of branch location
• Successful completion of nursing education from an approved school of nursing
• Five (5) years of clinical nursing experience encompassing a variety of clinical settings
• Three (3) years of home health experience preferred
• In-depth knowledge of homecare regulations
• Ability to independently organize work, provide appropriate case management, cost-effective home healthcare services, and timely, accurate documentation
• Strong critical thinking and communication (oral and written) skills and literacy in the English language
• Ability to use a personal computer with working knowledge of Word, Excel, Windows, and similar applications
• Minimum of twelve hours of continuing education annually
• Leadership experience preferred
• CPR certified
