Home Care Clinical Manager - RN

About the role

We are seeking an experienced Home Care Clinical Manager - RN to oversee patient services, coordinate care, and supervise staff in a home healthcare setting. This role demands independence, attention to detail, and commitment to quality patient care in a home environment.

Responsibilities

• Providing case management and ensuring proper documentation for patient care and staff performance.
• Supervising care staff and assisting the Administrator as needed.
• Delivering direct patient care when necessary, including care planning and staff assignment.

Qualifications and Skills

• Current RN licensure within the state, with a nursing degree from an accredited institution.
• At least five years of clinical nursing experience across various settings, with three years in home health preferred.
• Thorough knowledge of homecare regulations and effective organization skills.
• Strong critical thinking, communication, and documentation abilities.
• Proficiency in computer applications such as Word, Excel, and Windows.
• Minimum of twelve hours of ongoing education annually and CPR certification.
• Leadership experience is preferred.
