HOME CARE CLINICAL MANAGER - RN

About the Role

As a Home Care Clinical Manager, you will provide coordination and case management of all patient services, ensuring appropriate documentation for both patient care and staff performance.

Responsibilities

• Supervise assigned care staff and assist the Administrator as necessary.
• Provide direct patient care, including case management, OASIS care planning, assigning care staff, and ensuring efficient, effective utilization of existing staffing resources.
• Organize work independently, deliver appropriate case management, and provide cost-effective home healthcare services.
• Ensure timely and accurate documentation of care.

Requirements

• Current RN licensure within the state of the branch location.
• Successful completion of nursing education from an approved school of nursing.
• Five (5) years of clinical nursing experience across a variety of clinical settings.
• Three (3) years of home health experience preferred.
• In-depth knowledge of homecare regulations.
• Strong critical thinking, communication (oral and written), and literacy in the English language.
• Ability to use a personal computer with working knowledge of applications such as Word, Excel, and Windows.
• Minimum of twelve hours of continuing education annually.
• Leadership experience preferred.
• CPR certified.
