About the role
Support accurate reimbursement, regulatory compliance, and quality reporting by reviewing outpatient medical records and assigning accurate ICD-10-CM and CPT-4 codes. Success in this role is measured by coding accuracy, productivity, effective provider communication, and adherence to coding guidelines and documentation standards.
Responsibilities
- Review and interpret outpatient medical records to assign ICD-10-CM and CPT-4 codes with the highest level of specificity for ambulatory surgery, observation, special procedures, emergency department, ancillary, and clinic encounters.
- Code primarily complex ambulatory surgery and observation cases while maintaining a minimum coding accuracy rate of 95%.
- Abstract required clinical and demographic information into the electronic health record to support billing and reporting.
- Submit physician queries to clarify documentation and collaborate with clinical documentation specialists to ensure accurate coding.
- Identify documentation concerns, report risk management issues, and remain current on coding regulations, payer guidelines, and industry updates.
Requirements
- Current RHIA, RHIT, CCS, or CPC certification required.
- Minimum of 2 years of current full-time hospital outpatient coding experience involving ICD-10-CM, CPT-4, ambulatory surgery, observation cases, and medical record abstracting.
- Current AHIMA continuing education certification records required, when applicable.
