High Fidelity Wraparound Facilitator (TXFFPS)

About the role

The Harris Center for Mental Health and IDD (“The HARRIS CENTER”) is looking for a High Fidelity Wraparound Facilitator to join the Texas Family First Prevention Services (TXFFPS) team. Under the general direction of the High-Fidelity Wraparound Clinical Supervisor or designated supervisor for Texas Family First Prevention Services, this role provides direct ongoing intensive case management services through High-Fidelity Wraparound evidence-based principles and practices.

Responsibilities

• Collaborate, monitor and coordinate with Department of Family and Protective Services (DFPS) Child Investigators, Family Based Social Services workers, and Harris County Juvenile Justice Courts to develop and implement the best behavioral health High Fidelity Wraparound Plan for families.
• Perform clinical assessments to identify service needs and progress toward family treatment plan goals.
• Participate in the formulation of family wraparound plans and ensure the plan is written, authorized, and signed by an LPHA for consumers on caseload.
• Ensure all services provided meet eligibility criteria and are authorized in accordance with agency and regulatory guidelines.
• Provide skills training and psychosocial rehabilitation services addressing severe and persistent mental illness and other behavioral health symptoms that interfere with family functioning.
• Provide crisis interventions to reduce symptoms and avoid admission to more restrictive environments; collaborate with DFPS and Family Based Social Services workers to facilitate transfers to hospitals and outpatient services as indicated.
• Educate family members about youth and family needs (physical, social, and mental health) as identified by the plan of care.
• Assist youth and families in gaining and coordinating access to necessary care and services within the community based on mental, physical, and other social service needs.
• Provide information on identified behavioral health issues, the importance of medication and treatment compliance, and other clinical information to individuals, families, and community members as needed.
• Monitor consumer progress with treatment goals and current compliance with eligibility requirements (CANS/ANSA, financial assessment, current diagnosis) using monthly SAI reports and other available information.
• Meet divisional productivity standards as outlined in the mental health division’s operational guidelines.
• Adapt to changing work requirements and perform related work as assigned.
• Demonstrate ethical conduct in all clinical and professional activities and comply with agency policies, regulatory bodies, and performance contract documentation requirements.
• Attend and participate in team meetings with the clinical team.

Requirements

• A minimum of a Bachelor’s Degree in a Human Services field (e.g., psychology, sociology).
• Minimum of one (1) year of experience working in the mental health field, preferred.

Knowledge, Skills, and Abilities

• Basic understanding of mental illness, common signs and symptoms, and ability to identify difficulties with coping in community settings.
• Strong communication, listening, and interpersonal skills; written documentation is clear, concise, well-constructed, and grammatically correct.
• Ability to work collaboratively in a team environment.
• Effective time management and organizational skills.
• Ability to use standard word processing software on a computer.

Schedule

Flexible schedule with outpatient clinic; may require flexible working hours and travel. Coverage may be required in the absence of the practice manager, lead clinician, or rehab coordinator.
