About the Role

The Health & Wellness Specialist serves as the primary point of contact for clients, families, and community partners within CHOICE. This role is responsible for providing exceptional customer service, connecting clients to healthcare and community resources, and ensuring a positive client experience throughout the service process. The Health & Wellness Specialist works closely with case managers and service providers to coordinate referrals and facilitate access to essential support services. This position plays a critical role in helping clients access primary care, substance abuse treatment, insurance assistance, and other supportive services that promote overall health, stability, and well-being.

Responsibilities

  • Serve as the first point of contact for clients, caregivers, referral sources, and community partners.
  • Ensure client documentation is completed during office visits.
  • Assess client needs and connect individuals to appropriate services available through the agency's Resource Center.
  • Facilitate referrals and appointments with Primary Care Providers (PCPs), substance abuse treatment programs, behavioral health providers, insurance representatives, housing services, and other community resources.
  • Coordinate communication between clients, case managers, healthcare providers, and community agencies to ensure continuity of care.
  • Follow up on referrals and service connections to ensure clients successfully access recommended services.
  • Respond to client inquiries, concerns, and complaints in a professional, timely, and compassionate manner.
  • Provide information regarding agency programs, eligibility requirements, and available community resources.
  • Maintain accurate and confidential client records in accordance with agency policies, HIPAA regulations, and documentation standards.
  • Document client interactions, referrals, service outcomes, and follow-up activities in the agency database.
  • Monitor client service requests and assist in resolving barriers to accessing care and support services.
  • Collaborate with multidisciplinary teams to support client goals and improve service delivery.
  • Participate in outreach, client engagement, and quality improvement initiatives.
  • Maintain knowledge of community resources, healthcare providers, insurance programs, and social service agencies.
  • Perform special projects and other duties as assigned.

Qualifications

  • Associate or bachelor’s degree in human services, Social Work, Psychology, Healthcare Administration, Business Administration, or a related field preferred.
  • One to three years of experience in customer service, healthcare, social services, case management support, or client relations.
  • Knowledge of community resources, healthcare systems, insurance programs, and referral processes preferred.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to work effectively with diverse populations and individuals facing complex social, behavioral, or healthcare challenges.
  • Proficiency in Microsoft Office and electronic case management systems.
  • Ability to maintain confidentiality and handle sensitive information with professionalism.

Schedule

Monday – Friday, 9 AM–5 PM, 35 hours per week

Work Environment

Traditional office environment.

Pay

$46,000.00 - $52,000.00 per year
