Responsibilities
- Support outpatient rehabilitation through telehealth and connected care tools
- Coordinate virtual visits and onboard patients to digital platforms
- Monitor remote exercise and symptom data
- Ensure accurate documentation in electronic health records (EHR)
- Collaborate with PT, OT, and SLP teams to optimize workflows, improve access, and enhance patient outcomes
Qualifications & Skills
- Healthcare or digital health experience
