Position Summary

The Health Information Management (HIM) / Medical Records Coordinator is responsible for organizing, maintaining, and safeguarding resident health information while ensuring compliance with federal, state, and facility regulations. This role serves as a critical link between clinical, operational, and administrative teams by managing electronic medical records, coding, reporting, credentialing, and healthcare data systems.

Key Responsibilities

  • Maintain complete, accurate, and confidential resident medical records.

  • Review electronic health records (EHR/EMR) for accuracy and completeness.

  • Enter and maintain resident information within the EMR system.

  • Ensure compliance with HIPAA and all applicable healthcare regulations.

  • Manage medical records storage, retention, and retrieval processes.

  • Cooky Mountain storage schedules, shipments, and record retention.

  • Perform daily diagnostic coding.

  • Verify and enter classification codes for diagnoses, procedures, and treatments.

  • Add therapy codes weekly based on therapy logs.

  • Create and maintain diagnosis logs that transfer to UB-04 billing forms.

  • Participate in monthly Triple Check meetings.

  • Update and organize healthcare data within clinical databases.

  • Compile reports and data analyses as requested.

  • Monitor payer audits and denial trends.

  • Analyze clinical data to support quality improvement initiatives and reporting requirements.

  • Collect and document resident vaccination information upon admission.

  • Maintain vaccination status logs for residents and employees.

  • Manage and report vaccination and COVID-related data through the National Healthcare Safety Network (NHSN).

  • Ensure compliance with CMS, CDC, and state reporting requirements.

  • Verify required immunizations and COVID status through Florida SHOTS for staff, residents, agency personnel, physicians, nurse practitioners, hospice staff, and contractors.

  • Credentialing:

    • Coordinate credentialing and recredentialing of practitioners within the facility.
    • Maintain credential files and ensure compliance with facility requirements.

Qualifications

  • Previous Health Information Management or Medical Records experience required.

  • Skilled Nursing Facility (SNF) or Long-Term Care experience preferred.

  • Knowledge of medical terminology, diagnostic coding, and healthcare regulations.

  • Experience with EMR/EHR systems and healthcare databases.

  • Strong organizational, analytical, and communication skills.

  • Ability to maintain confidentiality and manage multiple priorities.

  • Proficiency with Microsoft Office and healthcare software systems.
