About the role

The Health Coach is part of the care team and under the supervision of the Director of Health Education. The Health Coach will work in conjunction with the Diabetes Self-Management team to assist the care team with direction and resources regarding diabetes self-management aimed at meeting the needs of the patients.

Responsibilities

  • Coordinate communication between patients, families, and caregivers with the multidisciplinary care team in order to identify barriers and support care plan.
  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions and self-management of diabetes into clinical practice.
  • Promotes self-management to achieve individualized behavioral and treatment goals.
  • Aids in meeting organizational goals.
  • Participates in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.

Requirements

  • Education: Associates Degree in Nursing Required or Bachelor Science in Nursing.
  • Experience: RN with one year or more related experience preferred.
  • Certifications: Current licensure as a registered nurse required by Arkansas State Nursing Board. BLS and certification required.

Qualifications

  • Computer Skills: Computer skills with capabilities to utilize Excel spreadsheets, graphs, and Word documents preferred.
  • Electronic Record Keeping: Ability to learn and utilize electronic record keeping systems such as electronic medical record required.
  • Physical Demands: Lift/Carry Stand O (Occasionally), Walk O (Occasionally), Sit F (Frequently), Handling / Fingering F (Frequently), Reach Outward O (Occasionally), Reach Above Shoulder O (Occasionally), Climb N (Not Applicable), Crawl N (Not Applicable), Squat or Kneel O (Occasionally), Bend O (Occasionally).
  • Other Requirements: Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI. Detail oriented, positive attitude, and the ability to take direction and work well with a team but also self-motivated. Excellent verbal and written communication skills. Must be able to work with a diverse patient population and have exceptional customer service skills. The ability to work in a constant state of alertness and in a safe manner.

Skills

  • Ability to communicate with patients and staff daily in a courteous and professional manner, address questions and direct calls promptly.
  • Serve as part of the care team in developing person-centered diabetes plans; follows and implements workflows that support and sustain MHSI Patient Centered Medical Home model of care.
  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.

Schedule

Positions Supervised: None. 8 a.m. to 5:30 pm Monday-Thursday. 8 a.m. to 12:00 pm on Friday.
