About the role

As a Health Coach at our organization, you will play a crucial role in supporting patients on their journey to better health. You will work closely with patients to develop personalized health plans and provide ongoing coaching and support.

Responsibilities

• Conduct initial assessments and develop personalized health plans for patients
• Facilitate regular coaching sessions with patients
• Monitor patient progress and make adjustments to plans as necessary
• Collaborate with healthcare providers to ensure seamless care coordination
• Stay updated on the latest health trends and research

Requirements

• Bachelor's degree in a related field or equivalent experience
• Strong communication and interpersonal skills

Qualifications
