Health Coach Educator

About the role

The Health Coach & Educator plays a crucial role in the Alignment Health team, dedicated to improving the health and wellbeing of seniors and those in need. This position involves conducting home and virtual visits to build a coaching relationship, providing support, education, and encouraging healthy habits.

Responsibilities

• Contacts patients eligible for Alignment’s Care at Home program and provides wellness coaching
• Collaborates with advanced practice clinicians and social workers to develop action plans
• Encourages adoption of habits conducive to a higher quality of life
• Delivers high-level patient service and increases member retention and satisfaction
• Aids patients in accessing community resources and social services
• Distributes health education materials to patients, caregivers, and families
• Coordinates care with other Alignment team members and health-related vendors

Requirements

• Minimum 1+ year of experience as a wellness or health coach or educator
• Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise
• Experience with applying the stages of change or similar behavior change models
• Excellent interpersonal skills and ability to foster trust and respect
• Independent and remote work capability
• Confidentiality and privacy in accordance with HIPAA regulations and AHP policies
• Proficient in Microsoft Office applications
• Effective written and oral communication skills
• Advanced problem-solving and organizational skills
