Health Care Management Nurse I/II

Job Responsibilities

  • Assist with the identification of the DDS service recipient's health risk by:
    • Completing a Physical Status Review (PSR) health acuity tool
    • Analyzing medical documentation and participating in service recipients' interdisciplinary team meetings to assist with the development of the Individual Plan.
    • C conducts home visits and reviews the home medical record for service recipients determined by PSR acuity score to have elevated health needs/risks.
    • Updates medical records in the consumer's electronic file.
    • Develops a Nursing Service Support Plan as indicated.
    • Calls with hospital medical staff on behalf of service recipients during hospital admissions to obtain updates on the individual's condition and to assist with planning for safe discharge.
    • Completes initial 24 Hour Mortality Report when notified of service recipient’s death.
    • May complete a Pre-admission Screening and Resident Review (PASRR) assessment for determination of admission eligibility to a Medicaid certified nursing facility for persons who may have an intellectual disability.
    • Sets up existing generic community health-related services to meet the service recipient’s health needs and actively develops (locates, contacts, and identifies) services when none are available.

    Knowledge, Skills, and Abilities (KSAs)

    • The knowledge, skills, and ability to analyze complex health concerns and issues, including the evaluation of adherence to established health care standards and the effectiveness of health management plans and/or programs, and recommending changes or revisions.
    • Employees have a high degree of independence in planning and completing work assignments and may function as the lead person on some special projects. Also requires professional nursing knowledge in reviewing, auditing, and analyzing nursing and healthcare services provided by direct care providers to ensure that the appropriate healthcare resources are used at the appropriate level to meet the healthcare needs of clients.

    Qualifications

    • Level I: Possession of a valid permanent Oklahoma license as a registered nurse, as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC), and two years of professional nursing experience.
    • Level II: Possession of a valid permanent Oklahoma license as a registered nurse, as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC), and three years of professional nursing experience.
