About the Role

The Health & Wellness Director (LPN) leads nursing services in assisted living and memory care communities, ensuring resident-centered, compliant care.

Responsibilities

• Supervise and coordinate all nursing activities, maintaining quality standards and regulatory compliance.
• Guide assessments, care planning, medication management, and staff training.
• Collaborate with leadership and interdisciplinary teams to foster a safe, home-like environment.
• Conduct resident assessments and ensure accurate documentation.
• Manage medication systems and support quality assurance and inspection readiness.

Requirements

• Current state Licensed Practical Nurse (LPN) license.
• Proven nursing leadership experience, preferably in assisted living, memory care, or long-term care.
• Strong clinical knowledge, leadership, and organizational skills.
• Excellent communication and teamwork abilities.
• Passion for senior care and alignment with core values.
• Dedication to high-quality resident care, sound judgment, and physical ability to assist with resident transfers and mobility.
