About the Role

The Health & Wellness Director (LPN) oversees clinical services in assisted living and memory care settings, ensuring resident-centered care and regulatory compliance.

Responsibilities

• Supervise nursing staff and maintain standards, policies, and procedures
• Conduct resident assessments and manage medication systems
• Collaborate with leadership and interdisciplinary teams for care planning and staff training
• Ensure a safe, respectful, and home-like environment for residents
• Support quality assurance and inspection readiness
• Promote resident safety and staff development

Qualifications

• Current state LPN licensure
• Proven nursing leadership experience, preferably in assisted living or memory care
• Strong clinical knowledge, decision-making, and teamwork skills

Physical Demands
