Health and Wellness Director (LPN) - Assisted Living

About the Role

The Health & Wellness Director (LPN) leads clinical operations within an Assisted Living community, ensuring resident-centered care and regulatory compliance.

Responsibilities

• Supervising all nursing services and staff, including LPNs, CNAs, and care team members.
• Overseeing assessments, care planning, medication management, and documentation accuracy.
• Implementing and updating nursing policies, standards, and procedures.
• Ensuring compliance with state, federal, and organizational regulations.
• Collaborating with leadership to coordinate staff training, orientation, and ongoing education.
• Promoting a safe, respectful, and homelike environment for residents.
• Supporting quality assurance and inspection readiness.
• Managing medication systems and resident safety protocols.

Requirements

• Current state LPN license.
• Proven nursing leadership experience, especially in assisted living, memory care, or long-term care.
• Strong clinical knowledge, organizational, and decision-making skills.
• Excellent communication, teamwork, and collaboration abilities.
• Passion for senior care and alignment with organizational values.

Physical Demands

Ability to lift up to 25 lbs., assist with resident transfers, and maneuver residents and equipment as needed.
