HC Meals Care Manager

What You Will Do

• Make home visits to assess needs.
• Develop appropriate service plans; coordinate and monitor services rendered.
• Advocate for consumers in obtaining benefits and services, and assist as appropriate.
• Maintain documentation.
• Initiate and maintain communication with appropriate community resources to better coordinate delivery of consumer services as needed.
• Conduct visits to meal sites; collaborate with meal site and other OCES staff.

What You Will Bring

• Bachelor's degree in Social Work or related field.
• Current social work license.
• PREFERRED: Bachelor's degree in Social Work or related field.
• Excellent verbal and written communication skills.
• Excellent interpersonal and conflict resolution skills.
• Excellent organizational skills and attention to detail.
• Strong analytical and problem-solving skills.
• Proficient with Microsoft Office Suite and other software.
