Responsibilities
- Implement habilitation plans for participants by reviewing records, observing and providing feedback to improve participants’ skills.
- Document site and community based activities.
- Assists and supports participants in developing and maintaining their independent functioning (including personal hygiene).
- Utilizes Protective Oversights to provide support to participants.
- Implements appropriate community based activities for participants.
- Trains and supports participants in performing volunteer activities in the community.
- Affords protection to participants during volunteer activities in the community.
- Oversees the safe arrival and departure of participants to and from the Day Habilitation site.
- Maintains an orderly work environment and keeps equipment and supplies in good condition.
- May act as an advocate to address concerns on behalf of participants’ rights.
- Transports participants to and from community outings and ensures their safety.
- Reports changes in participants’ physical condition to Day Habilitation nurses and supervisor.
- Performs additional duties as requested by supervisor.
- Ensures adherence to work-related safety procedures.
- Immediately reports unsafe acts or hazardous work environments to supervisor and/or safety committee.
Qualifications
- H.S. or Equivalent preferred.
- One year working with the developmentally disabled population preferred.
- Valid and qualified NYS driver’s license required.
- Good Oral and written communication skills.
- CPR/First Aid and defensive driving training preferred.
