Group Support Facilitator

About the role

Carda Health reimagines cardiac and pulmonary rehabilitation, offering it remotely to increase patient attendance and reduce costs. The company is led by Harry and Andrew, who founded Carda Health after experiencing family histories of heart disease. They work with major hospitals and insurers, backed by prominent investors.

Responsibilities

• Lead group support sessions according to clinical protocols
• Deliver high-quality care for patients in the cardiac rehab program and clinical studies
• Work across four elements: Nutrition, Stress Management, Group Support, and Exercise
• Collaborate with technology team to ensure a seamless patient experience
• Engage in a transdisciplinary team approach to support patient healing and connection
• Maintain a basic understanding of all four program elements to provide holistic patient education

Requirements

• Minimum 2+ years of relevant clinical or facilitation experience, preferably in behavioral health, cardiopulmonary, or chronic disease settings
• Licensed clinical social worker, licensed professional counselor, or other mental health professional
• Strong proficiency with mobile technology, including troubleshooting patients' technology issues with wearables and tablets
• Excellent interpersonal skills fostering a caring environment for patients
• Diligent and organized with the ability to create and adhere to repeatable processes
• Self-driven and able to perform with little direction in a high-growth startup environment
• Master’s degree in social work, counseling, or a related mental health field
• Bonus points for prior experience with Dean Ornish's ICR/OLM program, support group running, RYT, Registered Dietician, clinical trial participation, remote work experience, and education in health coaching
